In home care encompass all companion sitter duties and the following:

• Medication Management
• Vital signs (monitoring & recording blood pressure, pules/respirations, temperature, weight, and for diabetes finger sticks.
• Foley catheter care (measure input and output; emptyand clean)
• Colostomy Care (empty and clean)

In home non-medical care including:

• Light house keeping 
• Meal prep
• Providing companionship and social support
• Assist with activities of daily living: (bathing, dressing, toileting, grooming)
• Medication reminders
• Dementia & Alzheimer care
• Mobility assistance
• Parkinson care 
• Running errands/medical appointments
• Hospice/Respite care

As a result of over 25 years of case management experience, we are well versed in identifying the needs of patients. We offer assistance in coordinating care with all health providers and find community resources to improve patient outcomes. We work closely with hospitals, physicians, home health, and durable medical equipment companies to ensure continuity of care.

Discharge day can be hectic…. whether it is from a hospital, skilled nursing facility, or rehabilitation hospital. Being proactive and being prepared is key! 

We are able to communicate with the patient, family, health care provider, and home health during the hospital stay to initiate communication and care coordination to better engage patients and caregivers in the discharge plans.
